To understand Sculptra, it is worth first understanding what it is not. The most popular fillers — based on hyaluronic acid — work by directly restoring volume. The substance is placed beneath the skin and lifts it immediately. The result is visible straight away because the material physically occupies space. It is an effective and well-established technique, but it has one defining feature: it is external to the skin’s biology. Your body does not produce anything new. Material is simply added from outside.
Sculptra, or poly-L-lactic acid (PLLA), works in a completely different way. It is a biostimulator — its purpose is not to occupy volume beneath the skin, but to activate your body’s own collagen factory. Poly-L-lactic acid microspheres are injected into deeper layers of the skin and the tissue beneath it. They are too small to push the skin outwards on their own. But they are large enough for the immune system to detect and surround them. Through a low-grade inflammatory response, they stimulate fibroblasts to produce collagen intensively. What happens beneath the skin — step by step
Sculptra works in several stages, each taking place at a different time. This is why the result is not visible as soon as you leave the salon — the whole process is only beginning then.

Treatment day — injection. Diluted PLLA microspheres are placed in planned areas — usually deeper layers of the skin and around the subcutaneous tissue. A volume effect is visible immediately after the treatment. However, this comes from the carrier itself — the water used to prepare the product — not from collagen. This effect disappears within a few days.

step
First weeks — tissue response. Fibroblasts and the immune system ‘detect’ the presence of PLLA particles. A connective tissue capsule begins to form around each microsphere. This is the body’s natural response to biodegradable material. This quiet, controlled inflammatory response drives the entire process.

Weeks 4–8 — first results. Stimulated fibroblasts begin to produce new collagen fibres. The skin gradually becomes denser and firmer, with improved texture. The first signs of change are often subtle. People around you may start asking, ‘something has changed about you, but what?’ without identifying a specific treatment.

Why a series rather than one treatment
Sculptra is almost always used as a series of several sessions, usually spaced over time. This is not a sales-driven treatment that requires return visits for profit. It is a biological necessity that follows directly from how it works.
A single session starts collagen production in specific areas and to a specific level. But skin that has lost collagen over years needs more than one stimulus to rebuild what it lacks. Each further session strengthens the stimulus, reaches further layers and areas, and adds to collagen production. It is a little like physical exercise. One session does something, but only consistency brings real change.
Between sessions — and this is crucial — the work continues. Fibroblasts produce collagen for many weeks after the treatment, so the time between sessions is not a pause but an active building phase. Sculptra is not the right treatment for every case or every stage — and honesty requires saying this directly. It is a tool with a specific application profile.

- Thin, lax skin with reduced elasticity. When the skin has lost density and firmness, but the issue is widespread — there is no single wrinkle to fill, but an overall loss of quality — collagen biostimulation may improve the condition of the whole area.
