In women, melasma is usually hormonal in origin — it appears or worsens during pregnancy, with contraception, or with HRT. It can be extensive and symmetrical, and attempts to "mask" it with repeated peels often irritate it further. When it's worth it — indications

  • Melasma — symmetrical, brownish patches on the cheeks, forehead and above the upper lip; usually hormonal in origin.
  • Hormonal hyperpigmentation — after contraception, HRT, or pregnancy; often extensive and long-standing.
  • Sun spots and lentigo — the result of years of UV exposure, "age spots" on the face, hands, décolletage.
  • Uneven, "patchy" skin tone — skin with multiple small areas of hyperpigmentation from different causes.
  • Post-inflammatory hyperpigmentation (PIH) — dark marks left by acne, injury, or previous treatments; difficult for surface peels to address.
  • Hyperpigmentation that laser hasn't helped or has made worse — Cosmelan works enzymatically, not thermally, so it doesn't carry the risk of a reverse reaction the way light-based treatments can with melasma.

The hormonal balance of a woman's body is both beautiful and unforgiving to facial skin. Oestrogen stimulates melanocytes — the cells that produce pigment — which is why melasma is, in the vast majority of cases, hyperpigmentation seen in women. Hormonal contraception, pregnancy, hormone therapy during menopause, and even ordinary cycle fluctuations can activate pigmentation exactly where skin is thinnest and most exposed: the cheeks, forehead, temples, and the area above the upper lip.
This isn't neglect. It's physiology. And that's why exfoliation alone isn't enough.

Why women's skin reacts differently to men's

Women's skin is thinner, more hormonally reactive and more strongly predisposed to melasma — the deepest, most difficult type of hyperpigmentation, which reaches both the epidermis and the dermis. This depth means that a superficial peel removes only the top layer, and the pigment returns after a few weeks. Cosmelan works differently: it blocks tyrosinase — an enzyme activated by hormones and UV — before it can produce melanin. This is action on the cause, not just on the symptom.
The Cosmelan course is also planned with the sensitivity typical of women's skin in mind. The mask exposure time (8–12 hours), the concentration of the home course and the rate of intensification of Cosmelan 2 — all are selected individually, based on the phototype and reactivity of the specific person's skin, not on an averaged schedule.

A week that changes skin for over a year

Six days after the mask is applied, most women describe their appearance as intense. Skin turns red, peels in patches, and may feel like it's stinging. This isn't a side effect — it's the mechanism at work: the old, pigmented epidermis is shedding, revealing new skin underneath with reduced melanin production.
It is worth planning this week consciously — remote work, fewer meetings, phone calls instead of video conferences. Not because there is anything wrong with what is visible, but because the skin deserves calm during regeneration. And the final result — an even skin tone that those around you comment on: “you look great, something has changed” — is worth a few days in the comfort of your own home.

Skincare during the course — what stays, what goes

The Cosmelan course is six weeks of precisely planned care that requires one clear concession: active skincare ingredients step aside. Retinol, AHA and BHA acids, vitamin C in high concentrations — we suspend them all for the duration of the course. Not because they are harmful in themselves, but because Cosmelan 2 already provides the full spectrum of depigmenting action, and layering further active ingredients would disrupt the course and could irritate the skin at a critical moment.
What absolutely stays: SPF 50+ every morning, no exceptions, even on cloudy days. SPF isn't a bonus to the result — it's the condition for it. Melanin reactivates almost instantly under UV, even through cloud. The filter is literally what decides whether results last a year or return within three months.

Frequently asked questions

Can I have Cosmelan during pregnancy or while breastfeeding?
Unfortunately, no. Pregnancy and breastfeeding are absolute contraindications to Cosmelan — both because of the composition of the preparation and the increased hormonal reactivity of the skin at this time. Paradoxically, it is often pregnancy that causes melasma, but the treatment must wait until breastfeeding has ended. After that time, we plan a consultation and select a course of treatment — often the results are then very good.
I have an established skincare routine with retinol and acids — how does this fit with Cosmelan?
For the duration of the treatment (6 weeks), you give up retinol and exfoliating acids — not because they are harmful, but because Cosmelan 2 in itself intensively regulates cell renewal, and layering further active ingredients could excessively irritate the skin during regeneration. Once the treatment is complete, you return to your ritual — and it often turns out that active ingredients then work better, because the skin is more ready to receive them.
Can you wear normal makeup after Cosmelan?
Makeup isn't recommended during the peeling week — skin is in an intensive regeneration phase, and applying foundation or concealer could disrupt this process. After that week, makeup is fine, but SPF as a base remains the priority. As the home course progresses, skin becomes increasingly even — and many women find they need far less coverage than before the treatment.
Do the results look natural, or is it obvious something's been done?
Cosmelan evens out skin tone — it does not change facial features, structure or skin thickness. The results look like very good skin condition, not like an aesthetic intervention. Clients regularly hear: “something has changed about you, but I don't know what — you look great”. This is one of the reasons why Cosmelan is so popular among women who want improvement without a post-treatment look.
Can I combine Cosmelan with mesotherapy or Dermapen?
Not during the treatment course — these 6 weeks are exclusively for Cosmelan. However, once the protocol is complete, mesotherapy or Dermapen become an excellent complement: the active ingredients then penetrate deeper into freshly renewed skin, and the result is clearly better than if the treatments were performed separately. We plan such a schedule together, already at the consultation stage.
