When it's worth it — indications

Male Hair Loss — When Time Works Against You

In men, hair loss most often has a specific genetic-hormonal cause: male pattern baldness (AGA). A receding hairline, a thinning crown — this is an image most men recognise from their family or gradually see in the mirror. The mechanism is precise: DHT-sensitive follicles gradually miniaturise and produce increasingly thinner, shorter hair until they eventually stop. Crucially, this process begins long before it is visible to the naked eye. This is precisely why time is of the essence here.
ASCE exosomes are a signal preparation from stem cells. Through microneedling, they reach the scalp and transmit instructions to weakened, miniaturising follicles. These instructions include improving the microenvironment, supporting blood supply, and stimulating the growth phase. Where the follicle is weakened but still alive, exosomes make sense. Where the skin is already smooth and shiny, they will not recreate follicles. Therefore, timing is important, and trichoscopy provides an assessment of the follicle condition.

In men, scalp exosomes are primarily effective for:
  • early and moderate male pattern baldness — receding hairline, thinning crown (vertex);
  • thinning and softening hair — before miniaturisation ceases hair production;
  • strengthening the effects of other trichological treatments — for example, after PRP or in combination with mesotherapy;
  • overall scalp condition — for dryness, irritation, especially in men who wear hats or use strong styling products;
  • scalp care for short hair — short, cut hair does not hide scalp problems, and well-cared-for skin translates into healthier regrowth.

Exosomes vs. Other Options — What to Choose

Most men we speak with have done their research and come with the question: exosomes or PRP? The answer depends on the specific case, but the general logic is simple: PRP uses growth factors from your own blood. It requires a blood draw, and the result depends, among other things, on the quality of your plasma. Exosomes are a ready-to-use, standardised signal preparation that does not require a blood draw. Both mechanisms are different and often work best together, alternating in a series. Your trichologist will decide where to start after trichoscopy.
Minoxidil and finasteride? These are medications that act on the cause (hormones and follicle receptors). Exosomes support the growth mechanism; they will not replace pharmacology for advanced AGA. They can enhance it, but they are not an alternative to treating the root cause.

When It's Too Late — and When It's Not

The most common question men ask at their first visit is: is it too late? Trichoscopy provides the answer, not reflections in the mirror. Smooth, shiny skin without follicles is an area for hair transplant, not exosomes. However, many areas that appear bald still have miniaturising follicles. There is something to support there. That is why we do not answer this question without a trichoscopy camera.
The rule is simple: the earlier after symptoms appear, the more possibilities there are. Exosomes will not stop AGA permanently, but they can genuinely improve density and condition where follicles are still active. They can also slow down the rate of miniaturisation when combined with a trichological plan.

Frequently asked questions

Do I need to shave or cut my hair before the treatment?
No — we administer exosomes regardless of hair length. With short hair, it is easier to visualise thinning areas, but this is not a requirement. Your trichologist will adapt the technique to your hair length.
Is combining exosomes with minoxidil or finasteride safe?
Yes, there is usually no contraindication to combining it with topical or oral pharmacology used for AGA. However, please inform your trichologist about all medications and supplements you are taking during your consultation. They will assess the entire plan.
I have had a receding hairline for years, and it is already noticeable — does this make sense?
It depends on the condition of the follicles in that area, which trichoscopy will reveal. With a noticeable receding hairline and miniaturising follicles, there is something to support. However, if there is a complete absence of follicles, exosomes will not recreate them. We will tell you this honestly before you start a series.
How long do treatments need to be applied for the effect to be maintained?
Exosomes support follicles, but they do not switch off the AGA mechanism. After completing a series, we usually plan maintenance visits. Depending on your response and the rate of AGA progression, this could be every few or several months. Your trichologist will establish the plan based on post-series trichoscopy.
Can I have exosomes and colour or cut my hair at the same time?
Styling and cutting have no restrictions. It is advisable to postpone chemical colouring and strong styling products for a few days after the treatment. Your fresh scalp will be more sensitive then. Your trichologist will tell you exactly how long to wait in your specific case.
