Mechanism — how do they work?
- Method: Dermapen — Mechanism: Micro-needles create micro-punctures → controlled injury → fibroblast activation → collagen production — Depth: 0.5–2.5 mm (adjustable)Method: DermapenMechanism: Micro-needles create micro-punctures → controlled injury → fibroblast activation → collagen productionDepth: 0.5–2.5 mm (adjustable)
- Method: Non-ablative fractional laser (e.g. Pixel Er:YAG, ClearLift) — Mechanism: The laser creates columns of thermal micro-injury → coagulation → collagen renewal — Depth: 0.3–1.5 mmMethod: Non-ablative fractional laser (e.g. Pixel Er:YAG, ClearLift)Mechanism: The laser creates columns of thermal micro-injury → coagulation → collagen renewalDepth: 0.3–1.5 mm
- Method: Ablative fractional laser (CO2) — Mechanism: The laser vaporises micro-columns of tissue → deeper renewal → stronger effect — Depth: 0.5–2 mmMethod: Ablative fractional laser (CO2)Mechanism: The laser vaporises micro-columns of tissue → deeper renewal → stronger effectDepth: 0.5–2 mm

Key differences
Safety and skin phototype
Dermapen: Safe for phototypes III–VI (olive, dark and black skin). Micro-needles don't interact with melanin.
Fractional laser: Risk of PIH (post-treatment dark discolouration) in dark phototypes — especially with CO2. Mainly safe for phototypes I–III.
> A clear rule: dark skin → Dermapen. Phototypes IV–VI → consultation and caution with laser.
