What really happens to the skin during the treatment
Permanent Makeup for brows involves introducing pigment into the dermis — the layer beneath the epidermis. It is deeper than a standard cosmetic tattoo, yet shallower than a traditional artistic tattoo. This depth is deliberate. The pigment needs to be visible and long-lasting, while the skin retains its natural ability to remove particles over time. This is why the PMU result gradually fades and can be refreshed, rather than lasting for life.
The needle introduces pigment in small points and rhythmic movements, creating hundreds of micro-injuries. This is not damage in the everyday sense. It is a controlled stimulus to which the skin responds exactly as it is designed to: by initiating a repair cascade. This repair cascade is what you see over the following days — not a “temperamental” response from your skin.
The first hours and day one: intensity you did not expect
Immediately after the treatment, brows look noticeably darker and more defined than the result you discussed with your beauty therapist. This is not an error — it is physiology. The skin is slightly swollen, blood vessels are dilated, and the pigment is still close to the surface before it becomes “absorbed” more deeply. The colour you see immediately after the treatment is pigment in its raw state, before the skin applies its filters over the following days.
Mild redness around the brows, a feeling of tightness and sensitivity to touch may occur — this is a standard inflammatory response. The skin has just begun the first stage of healing: an influx of immune cells that assess the new substance. Some pigment that did not reach sufficient depth becomes trapped in serum, which will begin to form a delicate layer on the surface.
Days 2–4: scabbing appears, and that is good news
Between the second and fourth day, a thin scab begins to form on the surface of the brows — delicate and darker than the surrounding skin. At this stage, many people feel their brows look as though they have been drawn with an overly thick line, slightly “dried out”. This stage is often the most worrying, because visually it is the least similar to the final result.
The scab is the skin’s natural protection. The real repair work takes place beneath it. Fibroblasts produce new collagen fibres, epidermal cells migrate and restore the surface, while immune cells — macrophages — surround pigment particles and remove some of them. This final process — pigment phagocytosis — is one reason why PMU colour fades naturally over time.
Days 5–10: exfoliation and the great disappointment that is part of the plan
The scab begins to fall away — and with it, most of the visible colour also disappears. After exfoliation, brows look strikingly light for many people. It may seem as though the pigment has almost vanished, the outline has blurred, and the result is faint compared with what you saw immediately after the treatment. This is when some women panic and message their beauty therapist, convinced that “the treatment has not worked”.
But this is exactly the plan. Exfoliation removes the epidermal layer containing surface pigment — the unanchored pigment held in serum that did not reach sufficient depth. The pigment that will truly remain sits deeper in the dermis. It is now covered by a fresh epidermal layer. This new layer is initially thicker and less transparent than it will be later — which is why the colour beneath it is less visible.
Weeks 2–4: the colour returns — softer and more like your own
Sometime between day ten and day twenty-eight, something happens that is worth waiting for: the colour gradually returns. It does not return with the same intensity as immediately after the treatment. It is softer, gentler and more blended into the skin. This is how a PMU result should look: not like a painted line, but like part of your face.
During this time, the dermis completes its healing cycle. The epidermis matures and reaches its final thickness and transparency. Collagen at the sites of micro-injuries has already remodelled. The pigment that survived the immune response stabilises in the place where it will remain. What you see after four weeks is the most accurate preview of the final result.

A thin scab forms. Brows may look “dried out” and uneven. The real regeneration takes place beneath the scab.

The epidermis matures. Colour gradually “emerges” from beneath the regenerating surface, becoming softer and more natural.

Follow-up visit — why and when
The standard for PMU brows is a follow-up visit four to six weeks after the treatment. This is not an option for those for whom “something has gone wrong”. It is an integral part of the process, planned from the beginning. Only after complete healing can we objectively assess which areas retained pigment less well, where the outline is less defined, and what may need more colour.
Skin is dynamic and responds differently in different zones. The brow area closer to the nose often retains pigment differently from the tail of the brow. It would be a mistake to assess the result immediately after exfoliation and attempt to “correct” something that is still healing. A follow-up visit at the right time gives a complete picture — and allows the result to be refined with an understanding of how your individual skin responded to the pigment.
Aftercare that truly matters
The healing stage is the only time when you can actively influence the quality of the final result. Not through a special serum or expensive cream, but by avoiding several specific things and following the simple aftercare recommended by your specialist.
- Water directly on the brows during the first days — it may wash out pigment that has not yet had time to anchor.
- Sauna, swimming pool, sunbed for at least two weeks — heat, steam and chlorine accelerate exfoliation and may draw out pigment.
- Make-up around the brow area during the first weeks — there is a risk of introducing bacteria into healing skin.
- Picking the scab — even accidentally while washing your face — may cause areas of colour loss in the final result.
- Retinol creams and acids on the brow area — accelerate epidermal exfoliation, which shortens the longevity of the result.

When healing looks different — and when to get in touch
Most women go through brow PMU healing without complications. The stages described above — initial intensity, scabbing, fading and colour return — are the standard, not the exception. However, there are signs worth monitoring. In these cases, contact your specialist or doctor.
- Redness and swelling that increase after the third day rather than gradually settling — the standard inflammatory response peaks on the first day.
- A burning sensation or throbbing pain — slight sensitivity is normal, but increasing pain is not.
- Yellowish or greenish discharge — lymph fluid is clear or slightly straw-coloured. A colour change may indicate an infection.
- A rash or blisters beyond the brow area — may indicate an allergic reaction to the pigment or anaesthesia.
What affects the longevity of the result — and when to book a colour refresh
Brow PMU is not permanent for life. This is an advantage, not a drawback. The pigment gradually fades, allowing the shape and intensity to be adjusted as your style, face and preferences change. For most women, the result remains in good condition for one to two years. After this time, a colour refresh is worth considering.
- Skin type — oily and combination skin produces sebum more quickly. This may push pigment out of the dermis, so the result may fade sooner.
- Sun exposure — UV degrades pigment. People who spend a lot of time in the sun without protection may need a colour refresh sooner.
- Use of acids and retinoids — these accelerate epidermal renewal, shortening the time before visible fading.
- Hormonal balance and medication — some medicines and hormonal changes may affect the rate of epidermal renewal and, in turn, pigment longevity.
- Treatment technique and application depth — selected for your individual skin type by an experienced specialist.
