Why one word causes so much confusion
The problem starts with language. The word “surgical” suggests sterility and safety — and in a narrow sense, that is correct. Surgical steel used for operating instruments must meet specific standards for corrosion resistance and sterilisation. But this is not the same as a material that is safe during prolonged contact with living, healing tissue. A surgical scalpel remains in tissue for several minutes. Piercing or PMU jewellery remains there for months, sometimes years. These are two entirely different contexts — and require different standards.
Another source of confusion is online retail. Jewellery described as “hypoallergenic”, “surgical grade” or “medical” may be made from inexpensive alloys without any implant certification. This is not necessarily because the seller is lying. Often, they do not know what they are selling. These terms are not legally protected in many countries. They can appear on packaging without any obligation to confirm the composition.
Surgical steel 316L — what does it really mean?
Steel 316L, or its equivalent 316LVM — vacuum melted and vacuum purified — was for years the basic standard for body piercing. It is corrosion-resistant, relatively well tolerated by the body and considerably safer than cheaper mass-market alloys. When correctly manufactured and polished, it can be used as jewellery in a fully healed wound without major problems.
However, there is one important point: 316L steel contains nickel — an element responsible for one of the most common contact allergies worldwide. In people without an allergy, nickel bound within an iron alloy often causes no reaction. In people with an allergy — or those who develop one — it may cause redness, itching, delayed healing and recurring inflammation. A healing wound is particularly prone to absorbing metal ions. The risk is therefore higher than with intact skin.
Implant-grade titanium — why it is a different standard
Implant-grade titanium, most often labelled ASTM F136 or ISO 5832-3, is successfully used in joint replacements, bone plates and dental implants. These are situations where contact with tissue lasts for years or a lifetime. This is not a jewellery case study — it is the most extensively studied implant material in medicine.
From a healing perspective, piercing jewellery made from implant-grade titanium may be better tolerated than steel by sensitive people. For many, healing also progresses more smoothly. This is not a guarantee — every body responds individually. However, for a new wound, especially during the first weeks of healing, implant-grade titanium is considered the preferred standard by the professional piercing community (APP — Association of Professional Piercers).

“Titanium plated” and “PVD coating” — the myth of a glossy coating
This is one of the most common areas of confusion for clients — and sellers are not entirely without blame. Jewellery described as “titanium plated” or “PVD coated” (Physical Vapour Deposition — vacuum deposition) has a core made from another metal. It is often a cheaper alloy or even brass. A thin coating of titanium or gold applied through deposition may look identical to solid titanium jewellery, but does not have implant properties.
PVD coating is hard and resistant to wear during normal use. Yet in contact with a healing wound — where microtrauma, discharge and pH changes occur — it may crack or peel over time. This exposes a metal core that is not biocompatible. The result can be unpredictable tissue reactions, often mistaken for infection or an allergy to “titanium”. However, titanium itself is not to blame — there is simply very little of it, or none at all.
Gold, niobium, Bioplast — what else is worth knowing?
Besides titanium and steel, several other materials appear in the context of piercing and PMU jewellery. 14k or 18k gold — yellow rather than white, as white gold contains palladium or nickel — can be as well tolerated as titanium. It also suits a fine jewellery aesthetic. Importantly, it must be solid nickel-free gold, not gold plating. Niobium is less common, but has a biocompatibility profile similar to titanium and no sensitising potential. It is a good choice for exceptionally sensitive skin. Bioplast (PTFE) is a soft polymer used for nose piercings, labrets or pregnancy jewellery. It is flexible, colourless and well tolerated in many cases.

PMU and microblading — jewellery you cannot see
In permanent make-up (PMU) — brows, eyeliner and lips — questions about materials mainly concern needles and machine grips, not jewellery. During a PMU treatment, the skin is an open wound throughout the artist’s work. Needles and ring-shaped components that come into contact with tissue should be single-use and certified for medical use — with EO markings (ethylene oxide sterilisation) or radioisotope sterilisation markings.
However, there is one situation where jewellery directly matters for PMU: a piercing within the treatment area — for example, a nose piercing during nose liner PMU, a lip piercing during lip tattooing, or a brow piercing during microblading. During the treatment, the artist may ask you to temporarily remove the jewellery or replace it with a Bioplast retainer. This is not because it interferes technically, but to minimise the risk of microtrauma to neighbouring tissue and contamination of the treatment area.
How to identify implant-grade jewellery — a practical guide
The question “how can you identify a good material?” is simpler than it seems — although it requires a few steps. First rule: ask for the manufacturer’s specification or a material certificate. Reputable piercing studios and companies producing implant-grade jewellery (IMPLANT GRADE) — such as Anatometal, Industrial Strength, BVLA and Neometal — provide these documents without hesitation. Missing documentation is a warning sign, not a reason for the person asking to feel embarrassed.

Ask about the composition and standard
ASTM F136 (titanium) or 316LVM (steel) are specific standards — not “surgical grade” without a specification. A good piercer will tell you who made the jewellery.

Check the finish
Implant-grade jewellery has a polished, smooth surface with no visible microcracks, scratches or irregularities. Touch it — there should be no sharp edges.

Avoid coatings for fresh wounds
PVD, IP coating or anodisation on a core other than titanium — not for new piercings. Solid anodised ASTM F136 titanium is OK, as it is the same metal with a surface colour change.

The rule is simple: freshly pierced skin, the first weeks of PMU healing, re-intervention — only implant-grade material or solid nickel-free gold.

Once the wound has fully healed, you can choose high-quality 316L steel, gold plating from a trusted manufacturer or decorative jewellery — provided you have no allergies and maintain good hygiene.
What these myths really teach us
The story of “surgical steel” and “implant-grade titanium” is a good example of how medical terminology enters marketing language and starts to take on a life of its own — regardless of what it originally meant. The same applies to other terms used in piercing, PMU and even cosmetology: “natural”, “hypoallergenic”, “medical” — each of these words may mean a great deal or nothing at all, depending on who uses it and in what context.
Responsibility lies on both sides. A PMU specialist or piercer selecting materials for your treatment takes on the responsibility of knowing the standards and using materials that truly meet implant-grade criteria. A person who wants to know more — and asks about the composition of jewellery or needles — is not difficult or demanding. They are wise.
A healing wound is an open window into tissue — and what you allow through it matters. Implant-grade titanium, solid 316LVM steel and nickel-free gold are materials backed by decades of research and millions of applications. An eye-catching PVD coating and a vague “surgical” description are not.
