Most men notice hyperpigmentation later than women — they scrutinise their skin less often, use sunscreen less often, and rarely meet someone who tells them "it's time to deal with this". A scalp exposed to the sun, a face without SPF for years, shoulders and neck tanned with no protection at all — melanin gathers wherever it took the biggest hit. The result: uneven skin tone, dark patches, and in some spots something that looks like dirt but won't wash off. The good news: laser treatment was made for exactly this. Here it is, straight talk.

Where patches come from — short and honest

Hyperpigmentation is an excess build-up of melanin — a pigment the skin produces in response to UV damage. Simply put: the sun hits a cell, the cell defends itself with melanin, and sometimes it defends too hard or in the wrong place. The result: sun spots, uneven skin tone, and in older men, so-called liver spots — which have nothing to do with the liver. They got that name because people once linked them to ageing, not to the sun.

Men have thicker skin than women — that's an anatomical fact. Thicker skin hides the first signs for longer. Then, when something does appear, it's more visible and lasts longer, because repair mechanisms work more slowly. The scalp is a special case: the entire head, with no hair to shield it, tanned for years with zero protection. Most bald men develop hyperpigmentation earlier and more intensely here than anywhere else on the body.

Where it shows up most in men

There's nothing random about the location — hyperpigmentation appears wherever skin has received the highest cumulative dose of UV radiation and has the least protection.

  • Scalp and bald areas — the peak of exposure. No hair, no hat, every day, for years. Sun spots and age spots show up here most often — unevenly spread, varying in size, from light brown to dark brown.
  • Face — forehead, cheeks, upper lip — especially exposed in men, because the average guy reaches for sunscreen less often. The so-called sun mask happens less often than in women, but clustered spots and post-acne hyperpigmentation certainly don't.
  • Neck and shoulders — years at the pool, on the beach, on pitches, and driving with the window down. The neck gets sun even in the city. Patches here are often flat and widespread.
  • Backs of the hands and forearms — classic exposure from driving. In drivers and outdoor workers, the left side (in Poland) is often worse than the right.
  • Back and chest — in men doing physical outdoor work, or those who've gone shirtless in the sun for years.

How removal works — no mystery

Laser treatment for hyperpigmentation works on the principle of selective photothermolysis — technical-sounding, but the idea is simple: the light wavelength is chosen so melanin (the dark patch) absorbs it, while the surrounding tissue doesn't. The melanin heats up and breaks down, and the skin around it stays unharmed. Macrophages — the body's "clean-up" cells — clear away the debris over several weeks. The patch gradually fades.

What to expect in practice — from consultation to results

    Consultation

    We assess the lesions (type, depth, skin tone) and rule out anything that needs dermatological diagnosis. This is where we decide on the method and session plan.

    Preparation

    A few weeks before the first session: strict sun protection on the treatment area. The better prepared the skin, the better the result and the lower the risk of complications.

  3. 03

    Laser session

    We cleanse the skin and apply contact gel or cooling. Laser pulses feel like warmth or light snapping sensations. Duration depends on the size of the treatment area.

  4. 04

    The first few days after

    Skin may be red, slightly swollen, and sometimes small scabs appear. This is a normal reaction — don't pick or scratch. For 2–4 weeks, strict UV protection on the treated area is essential.

    Results after the full course

    Hyperpigmentation fades gradually — many people see a clear improvement after 2–4 sessions spaced a few weeks apart. We assess the final result 4–6 weeks after the last session.

One important point nobody mentions at the start: laser treatment removes what's already there, but it doesn't protect against new damage. If you go back to the same habits after treatment — no sunscreen, daily sun on your bald scalp — the patches will come back. Sun protection isn't a cosmetic whim. It's the condition for lasting results.

When to choose laser, and when something else

Laser treatment isn't the only solution — but for most sun-induced hyperpigmentation in men, it's the fastest and most precise. For the record, here's what else is worth considering, and when:

  • Chemical peels — a great companion to laser treatment, or a standalone option for milder, surface-level hyperpigmentation. Acids exfoliate the epidermis, even out skin tone, and improve texture along the way. Less often used alone for deeper, clustered patches.
  • Brightening therapies — creams and products with niacinamide, kojic acid, vitamin C. Useful as a complement to laser treatment and for maintaining results. On their own, they work slowly and usually aren't enough for intense hyperpigmentation.
  • Mesotherapy — strengthens skin from within, improving hydration and regeneration. A logical pairing with laser treatment: laser removes the hyperpigmentation, mesotherapy restores the skin's condition.

Laser + chemical peel

Laser targets deeper clusters of melanin, while the peel cleanses the epidermis and evens out skin tone. The combination works better than either method alone — especially for irregular hyperpigmentation on the face and décolletage.

Laser + needle mesotherapy

After a laser series, skin often needs rebuilding — hydration, a stronger skin barrier, regeneration support. Mesotherapy fills in what laser doesn't do: it supports the skin from within and extends the results.

Laser + dermatological consultation

Mandatory for lesions on the scalp, and anywhere patches are irregular in shape or changing over time. Dermatoscopy rules out lesions laser treatment shouldn't touch — this isn't a formality, it's a safety measure.

Questions men ask most often

One last thing: there's no point putting it off. Hyperpigmentation doesn't disappear on its own, and more sun keeps adding layers. The longer you wait, the more sessions you'll need, and the harder it gets to even out skin tone. This isn't an aesthetic matter you can leave for "someday" — it's a skin issue that needs attention before it grows.
