PMU removal: laser, remover, or just lightening before a touch-up?

Your brows are too cool-toned, but you still like the shape. You hear "laser" in one place, "remover" in another, and a suggestion to cover the colour in a third. These are not equivalent shortcuts.

PMU removal does not guarantee complete pigment clearance, should not begin without identifying the colour, and cannot fix scarring through lightening alone.

Laser is chosen after assessing the colour and pigment reaction

Laser energy is absorbed differently by individual pigments. Dark pigment may react differently than red, yellow, or white, and some blends turn into an unexpected shade after the first session. Therefore, a patch test and the product documentation, if you have it, are more valuable than a promise of a specific number of sessions.

Laser has the advantage when the pigment responds well to the available wavelength and the skin allows for a safe procedure. However, it is not automatically the best choice for every blend.

Remover works in a different way and also disrupts the skin

Chemical or saline methods do not "extract" pigment without a trace. They require controlled skin disruption and healing, which is why they carry a risk of scarring, discolouration, and texture changes. The product, technique, depth of the previous pigmentation, and the practitioner's experience all matter.

Remover may be considered for colours that respond poorly to laser or in specific areas, but the decision should not be based on the "no laser" slogan. No light does not mean no risk.

Lightening before new PMU is sometimes a better goal than full removal

If the shape is largely acceptable and the artist only needs a lighter base, full removal may be unnecessary. Agree on the final design before starting. This allows the person performing the removal to know which areas should disappear and which only need to lose saturation.

  • Laser: when the pigment and colour respond well to energy.
  • Remover: in selected situations after assessing the healing risk.
  • Lightening: when the goal is to make space for a new design.
  • No treatment: when the skin is actively irritated or the goal is not yet clear.

What PMU is in the skin — and why it does not behave like a tattoo

Permanent makeup (PMU) is a technique of introducing pigment into the upper layers of the dermis — shallower than a classic tattoo, but deep enough for the effect to last for years. The term "permanent" is slightly misleading here: PMU is designed to fade gradually. The pigments used in PMU are often chemically formulated differently than tattoo pigments — they have smaller particles, different dispersion, and react faster to laser radiation.

The problem arises when this fading does not go according to plan. Instead of disappearing gently, the pigment may: change shade (a classic example is brows that turned from dark brown to reddish or grey-green), "spread" and lose precise contours, or remain intense for many years, making it impossible to cover with new pigment without a visible colour conflict. This last scenario — old colour showing through the new — is the most common reason clients come for a laser consultation.

It is also worth knowing that PMU is not a uniform mass of pigment throughout the skin's thickness. The dye is distributed in space — some particles lie shallower, others deeper. Over the years, this distribution changes: some pigment is gradually "absorbed" by macrophages (immune system cells), and some migrates slightly in different directions. Every PMU is therefore different — and this means there is no single, predictable reaction to a laser.

How laser removes PMU — photomechanics in layers

A laser works on the principle of selective photothermolysis: it emits light pulses at specific wavelengths that are absorbed by the pigment (the target), not by the surrounding tissue. The energy of the absorbed pulse causes the pigment particle to heat up rapidly and break down mechanically — into fragments small enough for the immune system to gradually remove through the lymphatic network. The skin does not "burn" during this time — it is a precise, selective mechanism, not thermal destruction.

    The pulse hits the pigment

    Laser light is absorbed by pigment particles. The darker and more contrasted the pigment, the more intense the absorption.

    The particle undergoes photomechanical breakdown

    The pulse energy causes rapid expansion and disintegration of the particle into micro-fragments. This effect — called "photoacoustic" — is key in Q-switched and picosecond lasers.

    Fragments are absorbed by macrophages

    Immune system cells recognise the micro-fragments as foreign bodies and successively remove them through the lymphatic network. This stage lasts for weeks — hence the breaks between sessions.

    The pigment gradually fades

    With each session, there is less pigment. The rate of fading depends on the depth, the pigment colour, and the efficiency of the individual's immune system.

What is key in the context of PMU is that cosmetic pigments often react differently than tattoo pigments. Some of them contain ingredients that may darken instead of lighten under a laser — this applies especially to pigments containing titanium dioxide and iron oxides in specific forms. This is why a patch test — a small trial session in an inconspicuous area — is almost always performed for PMU before working on the entire area.

Lightening or full removal — they are not the same

This distinction is crucial — and often underestimated when planning a treatment. Complete PMU removal and PMU lightening for a new design are two different goals, two different protocols, and two different schedules. Confusing them at the consultation stage leads to frustration: an overly aggressive approach when working "for a new design" can damage the skin and prevent precise application of the new pigment; an overly conservative approach for intended removal unnecessarily drags out the entire process.

Lightening for a new design

when you want to change the shape, colour, or technique

  • Goal: to weaken the old pigment enough so it does not conflict with the new one
  • Number of sessions: usually fewer than for full removal
  • Required break before new PMU (min. 6–8 weeks after the last laser session)
  • Possible shape and colour correction — the new design is placed on a "cleared field"
  • The skin must be fully healed before the next pigment application

Full PMU removal

when you are not planning a new design or want complete clarity

  • Goal: maximum pigment reduction — to a natural skin appearance
  • More sessions, longer time intervals (the whole process can take many months)
  • Each session reveals deeper pigment fractions
  • The result is rarely 100% "zero pigment" — we speak of a noticeable range of reduction
  • A good choice when old PMU is asymmetrical, heavily colour-shifted, or after a failed touch-up

In practice, lightening the area for a new design is the preferred path — and often more cost-effective and time-efficient. Instead of aiming to eliminate the pigment entirely (which may require many sessions), we aim for a state where the old pigment is faded enough for the new application to cover it or build upon it without visible conflict. This is a subtle but important shift in perspective: we are not fighting the old PMU, but rather preparing a "background" for the new one.

What makes one PMU respond better than another

Not every PMU responds to laser in the same way — and it is worth knowing this before setting your expectations too high or too low. Several factors have a real impact on the pace and result of the process.

  • Chemical composition of the pigment. PMU may contain organic dyes, iron oxides, titanium dioxide, or mixtures thereof. Each reacts slightly differently to specific laser wavelengths. Organic pigments usually react quickly; metal oxides can be more resistant and may require testing several protocols.
  • Depth of implantation. The deeper the pigment, the more energy is needed to reach it — and the more sessions are required. PMU implanted more superficially (e.g., using the ombre technique on brows) often reacts faster than PMU from filling techniques, which reach slightly deeper.
  • Age of the PMU. Fresher pigment is usually more intense, but it also reacts vigorously to the laser. Old, faded PMU may already be partially "reduced" naturally — but its chemical breakdown is often less predictable.
  • Colour. Dark, cool shades (blacks, greys, cool browns) react faster — they absorb more laser energy. Warm, reddish dyes or mixtures can be more difficult to lighten in a controlled manner.
  • Layering. Many clients have PMU applied multiple times — corrected, adjusted, and covered by other technicians. Several layers of different pigments result in a more complex laser reaction that is harder to predict.
  • Skin condition and immune system. The speed at which the body removes broken-down pigment particles is individual. Healthy, physically active, and well-hydrated people often see faster progress.

Preparation and the process itself — step by step

Laser treatment for PMU does not start with a laser pulse — it starts with a conversation. The better we understand the state of the pigment, the skin history, and your expectations, the more precisely we can plan the entire process. Here is what the standard path looks like.

    Consultation and PMU assessment

    The specialist assesses the colour, intensity, age, and condition of the pigment, and asks about treatment history, medications, and sensitivities. Based on this, we plan the goal: lightening or removal, the estimated number of sessions, and the laser protocol.

  2. 02

    Patch test

    A small trial session on an inconspicuous part of the PMU area. It allows us to assess the pigment's reaction to a specific laser and settings — especially important for unknown or mixed pigments, PMU on eyelids, and fresh corrections.

  3. 03

    First sessions — working from the surface

    The laser breaks down the pigment fractions lying higher and more intensely. The area may look darker or more intense immediately after the treatment — this is a normal, short-term effect before the body begins to remove the fragments. Scabs may appear for a few days after the treatment.

  4. 04

    Breaks between sessions — essential biology

    The lymphatic system needs time to remove the broken-down pigment particles. Shortening the breaks reduces the effect and increases the risk of skin irritation. Sessions are usually scheduled every few weeks.

  5. 05

    Subsequent sessions — working with deeper fractions

    Once the surface layers of the pigment are significantly reduced, the laser reaches deeper. Here, progress may be slower and less spectacular between sessions — but it still occurs.

  6. 06

    Assessing the result and deciding on the next step

    When the lightening or removal effect is satisfactory, the skin rests and heals. For the "lightening for a new design" path — once fully healed, you can schedule a consultation with a PMU technician.

When laser makes the most sense — and when it is worth waiting

Laser is an effective tool for working with PMU — but it is not always the first step. Sometimes it is worth assessing first whether the problem requires a laser or if a technical correction is sufficient. Below is an outline of situations where laser gives good results for many people — and those that require a more cautious approach.

  • PMU with altered colour (e.g., brows in a grey-green or reddish shade) — the laser can selectively address the pigment fractions responsible for this shade before a new design is applied in the desired tone.
  • PMU with an asymmetrical shape — when correction with pigment alone is impossible or insufficient, the laser allows us to "erase" unnecessary fragments and draw a clean border.
  • PMU applied multiple times by different technicians — often the only way to "reset" the field before a new design.
  • PMU several years after application — the pigment is already partially reduced naturally, which can accelerate the laser effect.

How the laser works for PMU removal — without techno-jargon

The laser emits a precise pulse of light that penetrates the skin and hits the pigment grains. The pigment absorbs the energy, heats up, and breaks into smaller fragments — small enough for the lymphatic system to gradually remove them from the tissue. The effect is gradual: after each session, there is clearly less pigment, the contour fades, and the colour normalises. You do not touch the skin itself — the surrounding tissue remains intact.

PMU reacts to the laser slightly differently than a decorative tattoo because it is sometimes applied more superficially and with different pigments. This requires appropriate parameter selection — which is why a thorough interview and pigment assessment are conducted before the first session. Depending on the colour, depth, and saturation, the same device may require different settings for eyeliner than for brows.

Full removal or correction — two paths

Not everyone wants to remove everything. Often the goal is different: to lighten part of a contour that has blurred, to remove a colour that has turned green — so that a retouch can be done more precisely later. Or simply to lighten brows that are too dark, without erasing everything. Laser makes this possible.

Full removal

  • the brow shape is wrong and cannot be corrected
  • you want to return to a natural look
  • removal of PMU after an accident/surgery that was temporary

Selective correction

  • the contour has blurred, but the brow axis is good
  • saturation is too dark — you want to lighten it
  • preparation for a more precise PMU retouch
  • asymmetry — one side requires correction

In practice, most people who come to us with PMU issues do not want to "remove everything" — they want it to return to a state that does not draw attention. Often, a few sessions are enough for the pigment to fade sufficiently so it is no longer noticeable in daily life. Any planned retouching can then be performed with precision.

After removal — what next

Removed PMU is not the end of the road — it is a starting point for a decision you can now make without the pressure of existing pigment. Some people decide to have PMU again — this time more precisely and in the correct shape. Others prefer a natural look. Some choose to fill in their brows using a different method or decide not to return to the subject at all.

Laser scar therapy

If the PMU was a correction for a scar or asymmetry following surgery, it is worth assessing after removal whether the scar itself requires additional laser work.
