Trichology · an honest comparison of methods

Hair or blood? Elemental analysis does not answer the same question.

A good test starts not with a sample, but with the question the result is meant to answer.

If your hair has started falling out suddenly, elemental analysis of a strand should not delay a full blood count, ferritin levels, thyroid assessment or a medical consultation. A hair result is not a substitute for blood tests and does not diagnose the cause of hair loss.

It can provide additional data on elements detected in a specific sample, but interpretation is hindered by cosmetics, dyeing, the environment, the sampling site and differences between laboratories. Therefore, the comparison is not "which test is better". It is: "what question do I need an answer to, and which method has reliable clinical significance for it?".

Three methods look at three different parts of the problem

Hair elemental analysis, laboratory blood tests and trichoscopy are not three versions of the same test. Hair is a material in which a laboratory measures the content of selected elements. Blood allows for the assessment of specific body parameters according to medical reference ranges. Trichoscopy shows the scalp, hair shafts and follicle features under magnification.

Blood tests

  • thyroid parameters, inflammation or metabolism depending on your history
  • values interpreted in the context of symptoms and laboratory norms

Measuring an element in hair does not confirm a deficiency in the body

A laboratory can measure the content of selected elements in a sample. The most difficult stage begins afterwards: you must determine what a given value means for a specific person's health. For many elements, there is a lack of a stable correlation between the level in the hair and the level in the blood, organs or symptoms.

A systematic review of reference values indicated that the lack of generally accepted ranges makes it impossible to treat hair mineral analysis as a reliable method for assessing an individual's nutritional status. An expert panel from the American agency ATSDR also pointed to a lack of standardisation, external contamination, laboratory differences and the risk of misinterpreting the result.

Dye, shampoo and the sampling site can change the sample result

Hair remains in direct contact with water, air, dust and cosmetics. Dyeing, bleaching, mineral products, swimming pools and the work environment can affect measurements. Laboratories use sample washing procedures, but there is no single generally accepted way that always separates what is built into the shaft from what has settled from the outside.

  • The sample should be taken from a specific area and close to the skin, according to the laboratory's instructions.
  • The history of dyeing, bleaching and chemical treatments must be disclosed before collection.
  • Do not mix hair from different areas or with different histories just to obtain sufficient mass.
  • The report should state the analytical method, sample preparation rules and the laboratory responsible for the result.
  • Repeating the test in a different laboratory may give a different result, so a trend requires a consistent protocol.

The ATSDR summary of limitations lists, among other things, the lack of standard collection procedures, quality control differences and the difficulty of distinguishing external contamination. This does not mean that the equipment does not measure elements. It means that the number from the equipment and a clinical diagnosis are two separate levels.

Blood tests are better when you are asking about ferritin, the thyroid or anaemia

In the case of weakness, pallor, shortness of breath, menstrual disorders, weight changes or a sudden increase in hair loss, medical diagnostics are needed. A doctor selects tests based on your history — they may include a full blood count, ferritin, thyroid parameters or other indicators. There is no single universal "hair" panel.

In these situations, hair analysis can distract from the test that will actually change your treatment. If a doctor or specialist later deems an additional measurement useful, it can be considered as a supplement, not a gateway to help.

Trichoscopy is better when you need to see miniaturisation and skin condition

Elemental analysis does not show whether hair in a given area is becoming thinner, whether follicle openings are preserved, what the cuticle looks like or whether inflammation is developing on the skin. For such questions, an examination of the scalp image is used, and if necessary, a dermatological consultation.

Trichoscopy provides

Elemental analysis will not show

  • the causes of itching, pain or scaling

Hair analysis is only useful when addressing a narrowly defined question

Hair is a recognised material in certain toxicological and forensic applications, for example when testing for specific substances or particular exposures. However, you cannot transfer this reliability to every commercial nutritional panel. A method may answer one specific question well and another poorly.

  • The question should concern a specific element and a justified exposure, not a general "body balance".
  • The laboratory must have a validated method, quality control and clear interpretation rules for that application.
  • The result should be compared with your medical history, possible contamination and — if available — a confirmatory test.
  • Do not use it as a basis for diagnosing a disease, selecting treatment or starting supplementation without independent confirmation.

If an offer promises to diagnose stress, hormones, thyroid function, metabolic rate, diet and the cause of hair loss from a single sample, it promises more than the result can support. The broader the interpretation, the more important the question of evidence becomes.

The decision is guided by the symptom, not the convenience of sample collection

    Before purchasing, check the validation, reference range, collection procedure and what the report explicitly does not interpret.

Supplementation based on hair alone can create a new problem

Elements do not work on the principle that "more is better". An excess of some can be harmful or disrupt the absorption of others. A report with coloured fields and a ready-made list of supplements looks convincing, but it does not replace confirmation that you have a deficiency requiring supplementation.

Elemental analysis is not worthwhile for someone who expects a ready-made diagnosis or a safe supplement prescription from a single strand. Its limitations are too great for it to guide such a decision independently.

A normal number does not rule out the cause of hair loss, and a deviation does not name it

A result within the range printed by the laboratory may be reassuring, even though it has not examined iron stores, thyroid hormones or the appearance of the follicles. The opposite situation is just as misleading: a field marked in red may trigger a search for a deficiency, even though the deviation comes from a cosmetic, water, the way the sample was washed or a range adopted only by that specific laboratory.

Therefore, you must read the report on two levels. The first answers the technical question: what the equipment measured in this sample. The second is clinical: whether this number has a recognised link to the symptom and whether it has been confirmed by a method you can base a decision on. Moving between these levels requires evidence, not attractive graphics or a long list of ratios.

Good interpretation does not involve assigning meaning to every deviation. Sometimes the honest conclusion is that there is no basis for action. If, however, the symptoms are clear, a normal hair panel should not close the diagnostic process. The entire situation is what matters: the duration of the problem, the pattern of loss, skin condition, diet, medications, illnesses, events from recent months and results tailored to a specific suspicion.

The ability to test a hypothesis is also important. If the report suggests that a specific element is responsible for hair weakness, the person discussing the result should indicate how to confirm this thesis, what other explanations they are considering and how to recognise if the chosen direction was wrong. Without such a plan, a coloured table easily becomes a closed system: every symptom confirms the report, and a lack of improvement leads to further products. An auxiliary method should broaden the assessment, not make it resistant to verification.

First the clinical question, then the test material

If you want to check ferritin, test for ferritin. If you need to see the follicles, choose trichoscopy and a specialist's assessment. If there is a specific suspicion of exposure, ask a toxicologist about the correct method. Hair does not become a better material just because it is easy to collect.

Elemental analysis can be an addition to a well-posed question, but it should not invent one after you receive the table. This is a simple boundary: the result should support a decision, not create a problem that is then solved by a set of supplements. Before collection, therefore, write down one sentence: what you want to find out and what you will do if the result is normal or abnormal. If no answer reasonably changes your next step, the test is likely not the first tool you need. First, choose a question that actually requires an answer.
