The scalp — not like facial skin
The first thing worth understanding is that the scalp isn't simply skin with hair on top. It has its own physiology, distinct from facial skin or skin elsewhere on the body. Sebaceous glands here are far denser and more active than in most other areas. They secrete sebum at a rate that leads to rapid greasiness in many people — regardless of whether they have oily facial skin.
Sebum is joined by regular epidermal exfoliation: dead cells shed constantly, though often unnoticed. Under normal conditions, this is a healthy, natural process. The problem arises when these elements — sebum, dead skin, residue from styling products and shampoo — start to layer up and clog around the follicle openings. This creates something like a biological plug: an impermeable or poorly permeable layer that physically blocks access to the skin.
There's another factor too: care and styling products. Conditioners, hair serums, mousses, sprays — all leave residue on the scalp. Some ingredients are molecularly heavy and don't penetrate the skin. Instead, they settle on the surface, forming an additional insulating layer. For mesotherapy — which by definition aims to reach as deep as possible — this is a genuine obstacle.
What happens beneath the scalp — and what mesotherapy is targeting
To understand why cleansing matters, it helps to look one level deeper — at what mesotherapy is trying to achieve. The hair bulb lives within the hair follicle, which extends deep into the dermis. It's surrounded by blood vessels, fibroblasts and local immune cells. The condition of these structures determines whether hair grows strong, thin, or not at all.
Scalp mesotherapy delivers active substances directly to the vicinity of these structures — bypassing the epidermal barrier. In theory, this is an elegant solution: instead of waiting for an ingredient to penetrate the skin on its own (which often fails anyway), we deliver it exactly where it needs to act. But "direct" doesn't mean "unobstructed" — there's always some tissue between the needle and the follicle, and the quality of that pathway matters.

Bacteria, microbiome and inflammation — a quiet risk factor
There's another dimension often forgotten in discussions about cleansing: the scalp microbiome. Healthy skin is home to billions of microorganisms — bacteria, fungi, viruses. In balance, they form a protective barrier and cause no problems. But when the scalp is oily, has built-up dandruff, or shows inflammation, the microbiome can shift towards species that pose a risk during treatment.
This matters especially for people with seborrhoeic dandruff (pityriasis steatoides), seborrhoeic dermatitis of the scalp, or AGA (androgenetic alopecia), where the skin's condition already differs from normal. In these cases, cleansing isn't a cosmetic preamble — it's a condition for treatment safety.
Cleansing as a primer — not just dirt removal
It's worth shifting perspective: cleansing the scalp before mesotherapy isn't "washing just in case". It's active preparation of the surface for absorption. It works much like a primer before painting a wall — it doesn't just remove what's in the way, it creates a more uniform, receptive surface.

Cleansing also has a physiological effect: gentle massage and cleansing preparations increase local circulation. A better-supplied scalp means better-nourished hair bulbs and faster distribution of the delivered substance through the capillaries. This isn't a marketing claim — it's a simple consequence of the fact that heat and mechanical stimulation of the skin dilate blood vessels and speed up metabolite exchange.
When cleansing is especially critical
While cleansing is standard before every scalp mesotherapy session, some situations make it disproportionately important. These aren't edge cases — they're fairly common conditions clients bring to their appointments.
- Heavy styling and product build-up. Hairspray, mousse, serums containing silicones — each leaves residue that a few days of regular use can thicken into a layer that water alone can't remove. Enzymatic or gently exfoliating cleansing may be necessary here.
- Oily skin and dandruff. Excess sebum doesn't just create a mechanical barrier — it also alters the micro-environment on the skin's surface. For people with an oily scalp, cleansing is often deeper and takes longer than for those with dry skin.
- Long gap between washing your hair and your appointment. The standard recommendation is to arrive with a clean scalp — but not immediately after washing (skin needs time to rebuild its natural protective layer). The ideal state is a scalp washed a few hours earlier. This makes in-salon cleansing simpler and more precise.
- Active irritation or itching. Redness, flaking or an itchy scalp signals active inflammation. In these cases, the specialist assesses whether the treatment should go ahead at all — and if it does, cleansing becomes even more important as a risk-reducing step.
Cleansing and final results — does it actually make a measurable difference
This is a question worth asking directly: does skipping cleansing really change the outcome? There's no simple answer, because mesotherapy is a complex procedure, and results depend on many variables at once — protocol, substance composition, treatment frequency, follicle condition and overall health. No study isolates cleansing alone as a single variable.
But the mechanics are clear: a substance delivered to a precise depth, into well-supplied tissue, through clean skin, has a chance at full bioavailability. A substance delivered through a layer of accumulated sebum, with harder needle penetration and poorer blood supply — not necessarily. Across a series of sessions, where each builds on the last, differences in injection quality can accumulate visibly over months.
It's also worth looking at this from the perspective of a series. Scalp mesotherapy rarely produces dramatic results after a single session — its strength lies in regularity. If each of several or a dozen sessions starts with properly prepared skin, the cumulative difference is greater than the number of sessions alone would suggest. This is one of those elements that's hard to measure after one treatment but becomes visible over a six-month course.
